Dear Parent/Guardian of a school-aged child:
The State of Michigan requires students to be age-appropriately vaccinated to enroll in school programs,
unless a valid exemption applies. Parents must provide the school with the below vaccinations
and/or a valid exemption*.
The following vaccinations are required for students entering KD or a young five program.
5 doses DTap
4 doses Polio
3 doses Hepatitis B, or laboratory evidence of immunity
2 doses MMR, or laboratory evidence of immunity
2 doses Varicella, laboratory evidence of immunity, or disease history from a parent/ physician.
In addition to the above vaccinations 7
th
graders are required to have the following vaccinations:
1 dose ACWY Meningococcal Conjugate at the age of 11 years old or older
1 dose of Tdap at the age of 11 years old or older

1. *Non-medical Immunization Waiver The local health department must certify this type of waiver for
religious or other/philosophical objection(s) to vaccine(s). The required certification must be in person,
by appointment and available only at the Mount Clemens Health Center.
This type of waiver is now electronic and added to the students’ MCIR records by the issuing
Local Health Department. Parents are no longer required to submit a paper copy of this type of waiver
to the school.
To obtain the Nonmedical Waiver, parents/legal guardians are required to receive waiver education on
the vaccines waived and must schedule an in-person appointment by calling 586-466-6840.
2. *Medical Contraindication Waiver - This type of waiver is completed by a physician (MD or DO) stating
a medical reason that prevents the student from receiving a specific immunization(s) for a specific time.
To obtain the Medical Contraindication Waiver Form, the parent must get the form completed by the
student’s doctor and submit to their school.

Any child with a valid exemption to a particular vaccination is considered susceptible to that vaccine-
preventable disease and is subject to exclusion from the school if an outbreak of the disease occurs.
